De vissen laten bedanken voor de cholesterolremmers

Ons leefmilieu ‘verziekt’ letterlijk en figuurlijk. Zo bevat het zwemwater van vissen steeds meer medicijnen, zoals statines die hun spierkracht verlagen. De natuur biedt alternatieven, maar voor voeding als medicijn blijven in onze moderne samenleving slechts weinigen enthousiast.

Vissen verliezen aan maximale spierkracht als hun leefmilieu atorvastatine bevat. Ook spannen hun spieren zich met een vertraging aan. Dat blijkt uit proeven met de ‘grootbekforelbaars’, een exotische vissoort die voorkomt in Canada en Amerika. De studie is gepubliceerd in het blad ‘Environmental Sciences and Sustainability’. Atorvastatine is een ‘statine’: een remmer van de cholesterolaanmaak. Ze worden voorgeschreven om ons risico op hart en vaatziekte te verkleinen. Een belangrijke bijwerking van statines is spierpijn.

Nagenoeg tegelijkertijd met deze studie verscheen in het toonaangevend medisch tijdschrift ‘Circulation’ een indirect pleidooi voor het gebruik van nog meer cholesterolverlagende middelen. In een onderzoek met 5.291 personen die reeds een beroerte hadden doorgemaakt, bleek het nog verder verlagen van hun cholesterol het risico op hart- en vaatziekten en beroerte verder te verkleinen.

In Nederland werden in 2020 bijna 2 miljoen mensen met een statine behandeld. Dat is 1 op de 7 volwassenen. De huidige synthetische statines zijn afgeleid van ‘lovastatine’. Dat is een stof die van nature voorkomt in eetbare paddenstoelen (champignon, oesterzwam, cantharel, kastanjeboleet, shiitake) en schimmels, zoals die in ‘rode gist rijst’. Statines remmen het ‘sleutelenzym’ in de cholesterolaanmaak: de HMG-CoA reductase. De synthetische statines zijn zeer krachtig. Atorvastatine verlaagt in de hoogste dosering van 40-80 mg per dag het LDL-cholesterol met 50 procent of meer, tegen slechts zo’n 10-29 procent door 20 mg/dag lovastatine.

Statinegebruik verlaagt ontegenzeggelijk het risico op hart en vaatziekte, maar dat cholesterolverlaging de (enige) oorzaak is wordt betwijfeld. Statines hebben meerdere, ogenschijnlijk onafhankelijke, effecten. Eén daarvan is dat ze de mate van ontsteking verlagen. Ze veroorzaken een lichte ‘oxidatieve stress’ dat een belangrijk beschermingsmechanisme in ons lichaam activeert: het nrf2-systeem. Het gevolg is dat we daarna ‘beter tegen een stootje’ kunnen. Veel andere natuurlijke stoffen activeren datzelfde beschermingsmechanisme. Ze behoren tot de klasse van de ‘fytochemicaliën’, zoals resveratrol (druivenpitten), curmumine (geelwortel), sulforafaan (koolsoorten) en quercetine (uien, appels). Deze stoffen komen voor in groente, fruit en kruiden, onderdrukken (via nrf2) ontsteking, en hebben gunstige effecten op ons darmmicrobioom.

Onlangs verscheen een Amsterdams proefschrift waarin curcumine als supplement werd afgeserveerd. Het kon ‘onmogelijk werken’. Het onderzoek werd gedaan bij mensen met een chronische darmontsteking: de ziekte van Crohn en colitis ulcerosa. Ze kregen 8 weken lang 6 gram curcumine per dag, maar hun ontstekingsparameters veranderden niet. Dat kon echter ook niet, want die waren bij aanvang al ‘normaal’… Ook waren er geen veranderingen in het darmmicrobioom. ‘Dus konden die microben ook niet meer gunstige stofjes maken’. Deze waren echter niet gemeten… De boodschap werd kritiekloos door de universiteit en de pers verspreid en in bepaalde kringen gretig ontvangen.

Fytochemicaliën in groente en fruit lijken steeds meer te worden vervangen door medicijnen.

Dat veel geneesmiddelen zijn afgeleid van fytochemicaliën raakt op de achtergrond. Door te sleutelen aan lovastatine kon het farmaceutische laboratorium de huidige zeer krachtige statines maken. Daarmee corrigeren we op grote schaal de gevolgen van onze slechte leefstijl. In 2012 vond in Amerika tussen de 62.000 en 92.000 kilogram atorvastatine, en evenveel simvastatine, hun weg naar de gemeentelijke afvalwaterzuiveringsinstallaties. Aldaar wordt tussen de 85 en 90 procent uit het water gefilterd. Met de rest ‘behandelen’ we onze vissen, maar gelukkig horen we die niet over hun spierpijn klagen…..
